CAUSES OF HEADACHE IN LIVER FAILURE : pathlogical increase in ICP

Liver is important organ in our body, mainly by detoxification of elements in blood, so it prevents damage to other organs in body, most "selfless organ" in our body. it takes functions of detoxicification, production of pro and anti coagulants, serum proteins, transamination enzymes, and produces bile for fatty acid emulsfication and absorbtion

it also involves conjugation of indirect bilirubin to direct bilirubin and excretion of excess

Liver failure is loss of liver function due to infection i.e hepatitis virus, non alcohol fatty liver diseases, alcohol abuse, autoimmune diseases, medication excess i,e acetaminophen toxcity etc

SYMTOMS OF LIVER FAILURE:

  • yellow eyes and skin
  • upper left abdminal pain
  • a swollen body
  • nasuea
  • malaise
  • sleepiness
  • headache
COMPLICATIONS OF LIVER FAILURE:
  1. cererbral edema
  2. bleeding disorders
  3. fatigue
  4. renal disorders
  5. pheripheral edema
  6. portal hypertension
  7. hyperammonemia
PATHOPHYSIOLOGY OF CEREBRAL EDEMA:

Main pathology of increased intracranial headche comes from cytotoxci edema of cererbrum.

as hyperammonemia is common in liver failure, increased ammonia converts neuronic glutamate(which inturn comes from alpha keto glutarate) to "glutamine"
increased glutamine leads to toxicty and induses cellualr enlargement i.e cytotoxic edema

glutamine influx leads to cellualar edema

glutamate converting to glutamine due to hyperammonemia

 TREATMENT :
Treartment primarly follows to decrease the ammonia serum levels by decreasing protein diet. stoppage of drug that leading to toxicity like acteaminophen
long term treatment include liver transplant

And treament for acute ICP headche is use of osmotic diuretc like IV mannitol and loop diuretics like Furosemide, salient high ventilation, lumber puncture if severe

idiopathic ICP also causes from medications like tetracyclines, vit a excess, dapsone etc
